The ADRC (Aging & Disability Resource Center) requires all meal delivery volunteers to fill out a Confidentiality Statement and a Volunteer Application. These forms can be requested from Colleen Inman. The Volunteer Application asks for personal information, including a social security number, but it is not required. In addition, a copy of a driver's license and auto insurance is required. Two references are also needed and it is recommended that they be from club members. If a spouse or friend will also be delivering meals with you, they will also need to submit the paperwork.
