 uires all meal delivery volunteers to fill out a Confidentiality Statement

 man. The Volunteer Application asks for personal information\, including a
  social security number\, but it is not required. In addition\, a copy of 
 a driver's license and auto insurance is required. Two references are also
  needed and it is recommended that they be from club members. If a spouse 
 or friend will also be delivering meals with you\, they will also need to 
 submit the paperwork.
